2008 Registration Information

To make registration with Lebanon Little League as smooth as possible, we have provided the following forms for you to complete, print and take along with you to registration day.

Before registering for participation in Lebanon Little League, ALL Parents must read and agree to abide by the Youth Sports Parent Code of Conduct Contract by signing the bottom of the contract and submitting it along with their child's registration papers.

The Liability Release Form is the National Guard Armory waiver that every child needs to have filled out in order to practice in the armory.

The last page is the medical release. Please print this page and return it if your child has a pre-existing medical limitation. If your child does not have a pre-existing medical condition, you can simply indicate that in the space provided on the Registration and Permission Form.

Please remember to print each form by using your browsers print button before proceeding to the next as changes will be lost on exit.

This is the form you need only if you are renting the field for private use.
You may also print these forms and mail them in:
