2010 Registration Information

The following information is available to all potential Lebanon Youth Baseball Association (LYBA, Association) participants:

Regular Season Registration and Try-outs
(See separate "Summer Travel Team" registration forms to register for the summer travel team(s)).

In order to minimize the administrative burden and to assist the Association in ensuring a fair and orderly tryout and draft process, a registration deadline of March 1st has been implemented for the upcoming baseball season (May – June). Registrations will be accepted up to the established tryout date (TBD annually), however, registrations received after March 1st will be assessed a $25.00 late fee. Incomplete registration packets will be deemed late if not completed by March 1st. Players who have not turned in completed registration forms and/or payment (including alternative payment arrangements or scholarships) by the tryout date will not be allowed to try-out.

Registrations received after tryouts will be placed on a "regular season" stand-by roster. Players placed on the "regular season" stand-by roster will be allowed to play only in the event that space becomes available on one of the teams before the regular season begins. Players placed on the "regular season" stand-by roster are allowed to try-out for the Summer Travel Team in accordance with the "Summer Travel Team Registration and Try-out process."

Tryouts for A & B ball players:
Saturday, March 20th, 2010
at Lebanon High School Gym
A-Ball tryouts from 9am-10:30am
B-Ball tryouts from 10:30am-12pm

Regular Season Registration Package

The Registration and Permission Form (Required) must be completed in its entirety. The contact information must be legible. The contact information provided and the website are the primary sources of communication.

The grades listed on the registration form are to be used as a guideline for registration purposes. Players may be allowed to play or be “placed” outside of the grade recommendations (See Placement Form section below). This depends on many factors including but not limited to ability, safety and availability of space on teams. The Board reserves the right to make final determinations based on all relevant information available.

Fees for the upcoming season will be included on the applicable seasons registration form.

Concession Stand - Please note that it is an expectation that parents registering a player(s) in either A-ball or B-ball will share responsibility for staffing the concession stands. Also all players/families will be expected to participate in two fundraisers during the season unless the “opt-out” option is chosen (See Registration and Permission Form). The “opt-out” donation is in lieu of participation in the two fundraisers, it does not however absolve participation in the concession stand for A-ball and B-ball families. The concession and fundraisers are utilized to minimize increases in registration fees from year to year.

Youth Sports Parent Code of Conduct (Required) - ALL Parents must read and agree to abide by the Youth Sports Parent Code of Conduct Contract by signing the bottom of the contract.

Parental Waiver and Consent Form (Required) - The Parental Waiver and Consent Form must be completed for each player participating in the LYBA.

Medical Release Form (If Applicable) - Complete and return a Medical Release Form for each player who has a pre-existing medical condition. If the player does not have a preexisting medical condition, you can simply indicate that in the space provided on the Registration and Permission Form.

Placement Form (If Applicable) – The Placement Form is required if your child is no more than one grade level below the grade recommendation and you are requesting your child to be placed at the higher level (For example your child is in second grade, but you would like them to play B-ball). The Placement Form is to be submitted with all other registration forms by the required due date. The submission of a Placement Form does not guarantee that you child will be placed at the higher level. The Board reserves the right to place players at the appropriate level based on their discretion with all relevant information available.

Financial Assistance (If Applicable) – It is the mission and belief of the Lebanon Youth Baseball Association that every player should have the opportunity to participate in youth baseball. We have provided a discount for multi-player families and will also consider financial assistance upon request and approval by the Board of Directors or a delegated Committee of the Board of Directors. Requests should be submitted with the player’s registration forms, but no later than March 1st, so as to provide adequate time to review before tryouts.

Checks should be made payable to Lebanon Youth Baseball Association and mailed to:
